Will all my services be covered by my insurance?
Your insurance policy defines the benefits or coverage you are entitled to under your policy. Each plan is different. Your plan may cover all costs or some of the cost of your treatment or services less any deductible and co-insurance amounts, which are your responsibility. Your plan may not cover certain items such as take home prescriptions or certain treatments and conditions, for which you may be entirely responsible. Please refer to your policy or contact the claims administrator for the specific benefits.
Will Bryan Health file a claim with my insurance company?
Yes. We will file a claim with your health insurance company, Medicare, Medicare Supplement or Medicaid provided we have complete information. It is your responsibility to follow up with your insurance company after the claim has been filed. Remember, the insurance coverage is a contract between you and your insurance company.
If my insurance company disallows charges due to being over the "usual reasonable and customary charges", will I be responsible for those charges?
Yes. Some insurance companies use a reasonable and customary determination to base their coverage obligation rather than the actual charges incurred by their policyholders. The difference between the usual and customary amount and the charges shown on this statement are your responsibility. This limitation of coverage is based upon the contract between yourself and your insurance company.
