Survivorship care offers support, resources for ongoing symptoms and a focus on health and wellness so you can get the most out of life, as you move beyond your cancer treatments.
How can survivorship care help me?
Although treatment is over, you may have ongoing side effects, concerns and emotions. We work with a team of specialists to help you in areas that may be impacting your life and health such as:
- Fatigue, weakness, brain fog and pain
- Swelling in your arms or legs
- Anxiety, depression and stress
- Building strength and movement
- Finances, relationships and future plans
- Nutrition and healthy lifestyle
What can I expect?
At your first appointment, a nurse practitioner will visit with you about your health, symptoms and any issues you’re facing. We will work with you to create a plan based on your specific needs.
Your Survivorship Care Plan includes:
- A summary of all your cancer treatments (medication, surgeries, radiation)
- Recommendations for follow-up care and screening
- Healthy lifestyle education
- Strategies to manage long-term effects and symptoms
Your primary doctor and all specialists involved in your care will receive a copy of your plan.
Is survivorship care covered by insurance?
Most insurance plans cover this type of visit. We can help you determine if your plan covers this type of care.
