Your doctor may refer you to any or all of these health professionals to help you manage your diabetes. If you feel you can benefit from meeting with members of our care team, talk to your doctor or call us. We will coordinate any referrals needed with your doctor.
Diabetes Educator
Your diabetes educator will help you learn how to manage your diabetes through lifestyle, medication and new technology. The first meeting will be an evaluation to assess your overall health. The diabetes educator collaborates with you and your doctor to enhance your diabetes care.
Diabetes educators will:
- Provide ADA accredited education and support on nutrition, exercise, medication, coping strategies and more
- Train you on how to use an insulin pump and use advanced settings to manage your diabetes better
- Provide information on the benefits of Continuous Glucose Monitors (CGMs) and how they work as well as helping to determine what device is right for you
- Share your insulin pump and CGM readings with your care team
- Review education, medication and test results to help you manage your diabetes
- Provide nutrition counseling and education
- Provide gestational diabetes education, as needed
- Offer diabetes prevention education accredited by the CDC
Clinical Pharmacist
Our pharmacy team members are specially trained to evaluate medications and their role in improving your health. They partner with your provider and other care team members to help you meet you treatment goals.
Our pharmacist meets with you to:
- Make sure you are on the correct medications for your conditions
- Reduce out-of-pocket medication expense
- Help solve problems you are having with your medications
- Review lab test results and make appropriate medication changes
- Review the results of any tests you take at home (continuous glucose monitors, blood sugars, insulin pumps, blood pressure) and make sure your monitoring devices are working properly
- Answer questions you may have about your medications or health devices
- Help you keep a current medication list
Care Navigator Nurse
This specially trained nurse works with you to help you understand your health condition and improve your health. Together you will develop plans to meet your health goals based on your personal preferences and lifestyle. Care navigators partner with your provider and other care team members to help you meet you treatment goals.
Your care navigator provides:
- Education, whether you have a new diagnosis or have been living with diabetes for years
- Information on new approaches to manage your condition
- Support for other conditions that affect diabetes such as weight management, cholesterol, blood pressure and exercise
- Help with community resources
You can meet with your care navigator in person, by phone or through MyChart.
Social Worker
The social worker collaborates with you and your care team to help connect you to financial, food, transportation and other resources within our community. The Social Worker can help you find and access the resources you need.
Your social worker will help you with referrals and navigation of resources in the following areas:
- Medicaid/Medicare
- Health care financial assistance
- Economic assistance
- Transportation services
- Housing assistance
- Domestic violence and human trafficking
- Support with coping and adjustment needs
- Referrals to community resources and support
- Transitions to Assisted Living/Long Term Care
- Medical Power of Attorney/Advance Directives
- VA benefits
- Green card assistance
